AFFIDAVIT OF THOMAS W. LUKENS I, THOMAS W. LUKENS, am the Supervisor of the Quality Control Department at Zion Station.
I have held this position since February 1979, and I have been employed by Commonwealth Edison Company since July 1971.
I am a 1971 graduate of the College of DuPage in Glen Ellyn, where my major was metallurgy with minor concentration in quality control and nondestructive testing.
My entire employment with Edison has been involved with nondestructive examination.
I was first certified to perform Liquid Penetrant examinations, which is a particular discipline of nondestructive examination, on May 22, 1972 having met the standards of the American Society of Non-Destructive Testing, and I have been recertified every three years thereafter.
On December 12, 1978 I was also certified to perform visual inspections in accordance with ASNT standards.
As part of these certifications I have received yearly eye examinations.
My most recent eye examination was July 10, 1979, and at that time my vision was rated Jaeger-1, the best possible rating.
I performed visual and liquid penetrant examinations on Fuel Rack No. 1000083-2-1 on September 28, 1979 at Zion Generating Station.
The Commonwealth Edison Company, Special Process Prccedures Manual, procedure NDT-D, Revision 5 was used as the governing specification for the test method.

The first step of the testing was a visual examination of the ten areas to be inspected.
This was done in accordance to the checklist on the report form.
Flashlights were employed for increased illumination.
The liquid dye penetrant test involves cleaning the surface to be examined with an approved solvent, allowing this to evaporate for a specified amount of time, after which the dye is applied and allowed to soak / penetrate on the surface for fifteen minutes.
After which the dye is removed by the same approved solvent and the solvent allowed to evaporate.
The final step is spraying on the developer which then draws out any dye penetrant from surface cracks or voids.
The developer is a white chalky substance which will contrast greatly with the visible red dye penetrant. The record of these examinations is attached as Exhibit 1.
A sketch, similar to that shown in Ey.hibit 1, drawn by Station Nuclear Engineering Department (SNED) showed ten (10) areas to be examined which were selected by SNED as representative samples of structural support welds.
All areas examined were free of defect indications.
