Dear Mr. Grier:

This telegram is to confim the telecon between Mr. C. E. Hartman (THI-I Engineer III - Nuclear) and Mr. P. J. Kellogg (Region I - NRC) at 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br /> on 20 Septer:ber 1977.

A violation of the Technical Specification 3.6.1 occurred in that containment integrity was not maintained while the Reactor Coolant System

.was at 550 psig and 3500F.

At 1124 hours0.013 days <br />0.312 hours <br />0.00186 weeks <br />4.27682e-4 months <br /> or. 20 September 1977, while an employee was exiting the Reactor Building, both doors in the Personnel Access Hatch were open for approximately ten (10) minutes.

Irranediate corrective action was taken to shut the inner door from inside the Reactor Building. Af ter correcting the door interlock position, the outer door was closed. The door interlock function was then satisfactorily tested. In addition, a leak check on the door seals was satisfactorily per-fomed.

A more detailed evaluation as to the cause of this incident, and any long tem corrective action which may be necessary will be discussed in 7910250 7 7 3 .@
