Inadequate Administrative Centrc] of E=ergency Diesel Generater 13 Maintenance Type: Violatien of Technical Specificaticns, paragraph 6.2 3, which constitutes an abncr=al occurrence as defined by the Technical Specificaticus , paragraph 1.8g, in that the inadequate Administrative Centrol of E=ergency Diesel Generator 13 Maintenance =ay have threatened to cause an unsafe condition in the cperaticn of the plant.

[ Directorate of Licensing May 30, 197h PEZR Level: 120" PRZR Temp.: 6h6 F (6) Description of Occurrence: At 1900 cn May 19, 197h, during ner=al surveillance testing of the 13 Energency Dicsel Generator, it was discovered that the diesel engine would start as required but would not continue to run because of a 1cv lube oil pressure signal.

The diesel engine tripped en low lube oil pressure because three diesel engine lube oil pressure switches were disconnected frc=

their sensing lines. Investigation by the Plant Operations Review Cc==ittee (PCRC) revealed that the pressure switches had been mistakenly left disecnnected by =aintenance persennel on May 18, 197h, and that the maintenance Laing perfer=ed en the diesel engine at that time was not being dcne in accordance with an approved =aintenance procedure as required by the Technical Speci-ficaticcs and Station Ad=inistrative Precedures.

(7) Designation of Apparent Cause of Occurrence: Persennel (8) Analysis of Occurrence: The Energency Diesel Generatcr 13 was incapable of perfor=ing its design function for a period of approxi-

=ately h8 hcurs. This did not result in a threat to public health or safety because under the plant ecnditions that existed at the ti=e of thu occurrence, the diesels were not required to be operable in accordance with Section 3 7 of Technical Specifications. Further-

=cre, had a condition existed that required the diesel generators to be operable, Technical Specificatiens per=1t one diesel generater to be inoperable for seven days.

(9) Corrective Action: The Plant Operations Review Cc==ittee (PCRC)

=et prc=ptly after the incident and reccc= ended to the Plant Superintendent

a. a precedure to repair, test , and restere to service the Ener-gency Diesel Generater 13, and
b. that maintenance persennel involved in the incident be briefed en the i=portance of using approved procedures.

The Plant Superintendent concurred with PORC's recc==endations; the approved maintenance procedure was used to repair, test, 2nd restere to service Energency Diesel Generator 13; and =aintenance perscnnel were briefed on the i=pertance of using approved proce-dures.

In addition to the above acticas, the Statien Superintendent has instructed Statica F%nage=ent and Superviscry perscnnel to emphasize to these reporting to the= the i=pertance of perfer=ing vork in accordance with approved procedures. *
