P Appendix A NOTICE OF VIOLATION University of Wisconsin License No. 48-00361-14 License No. 48-09843-30 Based on the inspection conducted on March 7 and 8, 1979, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below.

Items 1 through 3 are infractions, and item 4 is a deficiency.

License No. 48-00361-14 1.

Condition 14 of License No. 48-00361-14 requires that you test your sealed source containing byproduct material for leakage and/or contamination at intervals not to exceed six months.

Contrary to this requirement, as of the date of this inspection, March 7 and 8, 1979, you had not tested the 2,000 Ci Cs-137 irradiator located in Room K-4358-C of the new University Hospital, for leakage since April, 1978, an interval of more than six months.

Condition 12 of License No. 48-00361-14 specifies that licensed material be stored under the supervision of James T. Rait.

Contrary to this requirement, from late December, 1978 through the date of this inspection, March 7 and 8, 1979, James T. Rait wes not affiliated with the University of Wisconsin and material was being stored under the supervision of another individual not named on the license.

10 CFR 20.203(c) requires that each high radiation area be con-spicuously posted with a sign or sigrs bearing the radiation caution symbol and the words:

" Caution High Radiation Area."

Contrary to this requirement, on March 7, 1979, a high radiation area in room K-4358-C of the new University Hospital was not posted aa required.

I Appendix A License No. 48-09843-30 4.

Condition 14 of License No. 48-09843-30 requires that after instal-lation of the irradiator and Cesium 137 or Cobalt 60 source and prior to initiation of the irradiation program, a radiation survey be conducted and a report of the results of the survey be submitted to the Commission within 30 days.

Contrary to this requirement, you completed the required survey on April 3, 1978 but did not submit a report of the results te the Commission until March 6, 1979, a period of more than 30 days. We note that this noncompliance was ideatified by you through your internal audit system.
