- 'A-NOTICE OF VIOLATION Geo-Engineering and Testing P. O. Box 3597 Agana, Guam 96910 License No. 56-18173-01 Based on the results of the NRC inspection conducted on December 6,1978, certain activities under your license appear to be in noncompliance with NRC requirements as identified below:
1.
Contrary to 10 CFR 19.ll(a) or (b), neither the documents required by 19.ll(a) nor the alternate notice permitted by 19.ll(b) were posted on December 6,1978.
(This item was corrected prior to the conclusion of the inspection and no written response is required.)
This is a deficiency.
2.
Contrary to 10 CFR 20.203(e), the laboratory room in which the Troxler Model 3411 density moisture gauge is stored was not posted with a sign bearing)the radiation caution symbol and the words:
" Caution (or Danger Radioactive Material (s)" on December 6, 1978.
This is a deficiency.
3.
Contrary to License Condition 13.A(1), the Troxler gauge containing cesium-137 and americium-241 sealed sources was put into use on December 6, 1978 without having received a leak test certificate from the transferor or a test for leakage by the recipient.
This is an infraction.
4.
Contrary to License Condition 16, which requires that licensed material be possessed and used in accordance with statements, representations and procedures in the application dated July 31, 1978, the nuclear gauge storage shed described in the application did not exist on December 6,1978.
This is an infraction.
