Nuclear Valve Division Borg-Warner Corporation Docket No. 99900289/78-02 NOTICE OF DEVIATIO:1 Based on the results of an NRC inspection conducted on December 11-15, 1978, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
A.
Criterion XII of 10 CFR 50 requires tools, gages, instruments, and other measuring and test devices used in activities affecting quality to be controlled, calibrated and adjusted periodically to maintain accuracy within necessary limits.
The foregoing requirement is amplified and extended by subparagraph 16.9.1 of the QA Manual which states that the QA Manager is to be notified when discrepancies in inspection equipment are found calibration and all materials and items inspected with the discrepant equipment, si..ce the date of the previous valid calibration, are tagged " Hold for Disposition" by the QC personnel.
Contrary to the above, the QA Manager was not notified when three (3) personally owned micrometers used for final acceptance were found to be outside the necessary limits of accuracy, and the items inspected with the discrepant devices were not being tagged and held for acceptance verification.
B.
Criterion V of Appendix B to 10 CFR 50, and article NCA 4134.5 of Section III of the ASME Code, state in part, " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings... and shall be accomplished in accordance with these instructions. Section 13.0, subparagraph 13.9(b) of the QA Manual states in part, "the Welding Procedure Specification (WPS) of the
'rrect revision is at the work station."
Contrary to the requirements of the QA program commitments, the WPS 5-3, Revision G identified on the shop traveler (s) for work orders 56820, 57680 and 52837 were not available to the weldors at three (3) of the work stations inspected.
C.
Criterion V of Appendix B to 10 CFR 50, and article NCA 4134.5 of Section III of the ASME Code, state in part, " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings... and shall be accomplished in accordance with these instructions."
In addition Section 13.0 subsection 13.1.4 of the QA Manual states that " Welding scall be performed... in accordance with qualified WPS's."
790306OW Contrary to the above requirements, it was observed at one (1) work station, the seal welding operation being performed on work order (WO) 053412-40, using Welding Procedure tiPB-5-1-Revision G was not being complied with as required by the WPS.
WPS 5-1-Revision G regt. ires that the electrical characteristics be as follows, 100-150 amperage, using 1/8" Electrode. The inspector observed that the amperage being used was 80 amps and the Electrode size was 3/32.
