Inspection :) August 8-10, 1978 (99900228/78-01)
Areas Inspected: Action on five (5) previously identified inspection findings, implementation of 10 CFR 50, Appendix B criteria, and applicable codes and standards, including document control, inspection, and test control. The inspection involved twenty-four (24) inspector hours on site.
Results:
In the three (3) areas inspected. the following deviations and unresolved item were identified:
Deviations: Document Control - system and practices were not consistent with
(*iterion VI of Appendix B to 10 CFR 50 (Enclosure, Items A. and B.}
Test Control - practices were not consistent with Criterion XI of Appendix B of 10 CFR 50 (Enclosure, Items C. and D.).
Unresolved Item:
Inspection - procedure identifies specific forms to be used, however, fonns do not exist (Details Section D.3.b.).

(Closed) Deviation (Report No. 77-01): The inspector verified that Section 2.2 of the Quality Assurance Manual had been revised to delineate in writing the authority and duties of the Engineering, Manufacturing, Sales, Purchasing, and Production Control Departments.

(Closed) Deviation (Report No. 77-01):
The inspector verified that Factory Instructions Nos.142 and 070 had been revised and initiated, respectively, to assure translation of contractual data into company data and for selection and review of material, parts, equipment, and processes.

Objectives The objectives of this area of the inspection were to verify that:
a.
Measures had been established to control the issuance of documents, such as instructions, procedures, and drawings, including changes thereto, which prescribe all activities affecting quality.
b.
Established measures:
(1) Assured that documents, including changes, are reviewed for adequacy and approved for release by authorized personnel.
(2) Assured that documents, including changes, are dis-tributed to and used at the location where the prescribed activity is performed.
c.
Changes to documents had been reviewed and approved by the same organizations that performed the original review and approval or another designated responsible organization.
2.
Methods of Accomolishment The preceding objectives were accomplished by:

Review of the following customer orders and attendant documents to verify that document control had been irvoked:

Obj ectives The objectives of this area of the inspection were to verify that:

A program for inspection of activities affecting quality had been established and executed to verify conformance with the documented instructions, procedures and drawings.
b.
Performance of inspection was accomplished by individuals other than those who performed the activity being inspected.
c.
Examinations, measurements, or tests of material or products were performed for each work operation where necessary to assure quality.
d.
Indirect control by monitoring processing methods, equipment, and personnel had been provided where inspection was impossible or disadvantageous.
e.
Both inspection and process monitoring had been provided when control was inadequate without both, f.
Mandatory inspection hold points had been indicated in appropriate documents and observed.

Unresolved Item Specific forms are required during preliminary end examination along with the requirement for the operator's signature in the appropriate space.
However, forms of the particular designation do not exist. Further, the forms in use have no provision for signature of the operator.
The contractor needs to align his procedure and practice.

Objectives The objectives of this area of the inspection were to verify that:

A test program had been established to Jemonstrate that structures, systems, and components wo'Jld perform satis-factorily in service and tests were pr.rformed in accordance with written test procedures which ini:orporated the require-ments and acceptance limits contained in applicable design documents.
b.
Test procedures included provisions for assuring that all prerequisites had been met, that adequate test instrumentation was available and used, and that tests were performed under suitable environmental conditions.
c.
Test results had been documented and evaluated to assure that test requirements had been satisfied.
2.
Methods of Accomplishment The preceding objectives were accomplished by:
a.
