Appendix A NOTICE OF VIOLATION Toledo Edison Company Docket No. 50-346 Based on the inspection conducted on November 1, 2, 5 thru 10, 12 thru 16, 19 thru 21, and 26 thru 30, 1979, it appears that certain of your activities were in noncompliance with the NRC requirements as noted below. This item is a noncompliance of the infraction level.
Technical Specification 3.4.1.a, requires that in modes 1 and 2, with one reactor coolant pump not in operation, STARTUP and POWER OPERATION may be initiated and may proceed provided THERMAL POWER is restricted to less than 78.3% of RATED THERMAL POWER and within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> the setpoints for the following trips have been reduced to the values specified in Specification 2.2.1 for operation with three reactor coolant pumps operating:
- 1. High Flux
- 2. Flux-Flux-Flow The surveillance portion of that specification requires that the Reactor Protective Instrumentation channels specified be verified to have had their trip setpoints changed to the values specified in Specification 2.2.1 for the applicable number of reactor coolant pumps operating...:

Contrary to the above on November 26, 1979, while in Mode 1, reactor coolant pump 1-2 was tripped and the High Flux trip setpoint was not changed to the value specified in Table 2.2.1 within four hours after the change to 3 RCP operation. The Flux-Flux-Flow trip setpoint was not verified to have its setpoint changed to the value specified in TS 2.2.1 within four hours after the change to 3 RCP operation.
