99900378/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on August 8-9, 1979, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR 50 states, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

A.

QA Procedure No. QA-2-101, paragraph 6.2 states in part, "... Purchase Orders shall include.the following requirements: NAMCO vendors shall submit a Certificate of Conformance for that material which is critical to the function of Nuclear Limit Switches.

Contrary to the above, certain NAMCO purchase orders for material critical to the function of nuclear limit switches did not require a Certificate of Conformance, example:

P.O. Nos. E-6390 and E-6312, each dated July 31, 1979.

QA Procedure No. QA-2-101, paragraph 8.0 states in part, "The following equipment used in inspection and testing shall be calibrated: Calipers and Micrometers, Plug and Thread Gages, Pressure Gages, Spring Tester, Surface Plate, and Gauge Blocks."

Contrary to the above, the equipment listed had not been calibrated although they were being used in the inspection and testing of products manufactured under the QA-2-101 program.

QA Procedure QA-2-101, paragraph 11.2, Training, states, " Periodically (quarterly minimum) a training session, with mandatory attendance shall be held for all personnel involved in the assembly, testing, and inspection of Nuclear Limit Switches. Those in attendance shall be advised of the special requirements of the Quality Assurance Program as applicable to the Nuclear Limit Switches. These training sessions shall be documented with respect to date, attendees, and agenda."

Contrary to the above, no training sessions had been conducted and/or documented since the publication of the QA-2-101 procedure on February 7, 1979.
