APPENDIX A NOTICE OF VIOLATION Connecticut Yankee Atomic Power Company Docket No. 50-213 Based on the results of an NRC inspection conducted on July 17-20,1979, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Facility License No. DPR-61 as indicated below.
Both items are deficiencies.
A.
Technical Specification 6.8.1 states, in part:
" Written procedures and administrative policies shall be established, implemented and maintained..."
Centrary to the above, the following examples of failure to implement plant procedures were identified duVing this inspection.
1.
On March 4, 1979, several data sheets from Surveillance Procedure, SUR 5.3-6, Revision 6, dated February 27, 1979, were not completed in that all of the required plant parameters were not recorded on the data sheets.
2.
On February 27, 1979, numerous changes to valve checklists in Nonnal Operating Procedure N.O.P. 2.4-1, Revision 3, dated November 23, 1977 were made without initiating a temporary change request.
In addition, step 7.1.2.7 of the procedure did not contain operator initials signifying completion of this step.
3.
On March 4, 1979, step 7.6.1 of Normal Operating Procedure N.O.P.
2.1-2, Revision 4, dated August 25, 1978 was not completed in that the required information was not recorded in the procedure.
B.
Technical Specification 6.8.3 states, in part: " Temporary changes to procedures... may be made, provided:
"The change is documented, reviewed by the Plant Operations Review Committee (PORC) and approved by the Plant Superintendent within 14 days of implementation."
Contrary to the above, on February 27, 1979, a temporary procedure change was made to Normal Operating Procedure, N.O.P. 2.4-1, Revision 3, dated November 23, 1977, Filling and Venting of Entire System, while the procedure was being used, without-subsequent review and Plant Superintendent approval as of July 20, 1979.
