Appendix A NOTICE OF VIOLATION Docket No. 50-546 Public Service of Indiana Docket No. 50-547 Based on the results of NRC inspections conducted June 9-July 7, 1979, it appears that certain of your activities were not conducted in full compliance This item is an infraction.

10 CFR part 50, Appendix B, Criterion VII, states in part that " Measures and shall be established to assure that purchased material, equipment, services, whether purchased directly or through contractors and subcontractors, conform to the procurement documents

. Documentary evidence that material and equipment conform to the procurement requirements shall be available at the nuclear power plant site prior to installation or use of such material or equipment.

The Marble Hill PSAR, Chapter 17.1.7, Section 17.1.7.1 requires, in part, that " PSI assures that material, equipment, and services procured by and Measures.

Contrary to the above, on and about June 27, 1979, nondestructive testing (pulse echo) of concrete structures by the site civil contractor was initiated, although the following documentary evidence of conformsnce was either unavailable or inadequate:

Calibration records for the electronic instrumentation.

Personnel qualification records for a pulse echo test technician.

Moreover, documented procedures or instructions for conducting, controlling and evaluating this nondestructive test activity, were either inadequate or unavailable.
