Appendix A NOTICE OF VIOLATION Cincinnati Gas and Docket No. 50-358 Electric Company Based on the results of an NRC inspection conducted on April 25-27, 1979, it appears that c.ertain of your activities were in noncompliance with NRC requirements as noted below. This item is a deficiency.
10 CFR 50, Appendix B, Criterion IX, states, in part: " Measures shall be established to assure that special processes including welding... are controlled; and accomplished by qualified personnel using qualified procedures."
Wm H. Zimmer Nuclear Power Station, Unit 1, FSAR, Chapter 17, Paragraph 17.1.9.1 states, in part:
"Special processes are accom-plished and controlled by qualified personnel."
KEI Special Process Procedure No. 8, Rev. 2, Appendix B, states that the inspector:
"Will stamp and date his acceptance of indicated requirements."
Contrary to the above, on April 25, 1979, the inspector observed that welding on Field Weld Nos. 8, 9, 10, 11, 12 and 13 of a steam line flailing restraint inside of the drywell had proceeded even
- t. hough the inspector had not stamped his acceptance / rejection of proper welding procedures. welder qualifications, filler material joint design / cleanliness, and fit / tack welds.
