Dear W. O'Reilly:

On May 21, 1979, a report was made under the provisions of 10CFR50.55(e) concerning 2ngle iron braces located on the exhaust ducting of the diesel g w rator cooling fans.

The angle iron braces were tack welded diagonally across each corner of the ducts, in acccedance with the reporting requirements of 10CFR21, the following info m tion is submitted:

Description of detect, deficiency, or f ailure to comply:

Angle iron braces installed to aid in the f3brication of the diesel generator cooling f an exhaust ducts were inadvertently left in the installed exhaust ducts.

These braces were tack welded and were not g

designed to remain in place once the ducts were installed.

The rs0 traces were loc ated cbove the diesel generator cooling f ans.

Failure g h O of the angle iron brace tack welds could have possibly allowed the

braces to f all into the ccoling f ans, causing damage to the f ans and possibly rendering the diesel generator incoerable.
