This refers to the QA Program Inspection conducted by Mr. J. R. Costello of this office on December 4-8, 1978 of your facility at Chicago, Illinois and to the discussions of our findings with Mr. J. E. McFarland and members of your staff at the conclusion of the inspection.
Areas examined during the QA program inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of p' adures and renresentative records, interviews with personnel, and ct.

During this inspection it was fount chat the implementation of your QA program failed to meet certain NRC requirements. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
