Dased on the results of an ilRC inspection conducted October 23 through fiovember 19, 1973, it appears that certain of your activities were not conducted ir full compliance with conditions cf your Operating License I;o. IlPF-6 as indicated below:
1.
Techn4 cal Specification 3.7.11 requires that if a penetration fire barrier is nonfunctional, a continuous fire watch be established on at least one side of the af fected penetration within one hour.
Contrary to the above, on October 24, 1978 during the evening shift, the inspector found a nonfunctional penetration fire barrier in the 2A3 switchgear room for which a fire watch had not been established.
A temporary cable had been passed through the penetration.
This is an infraction.

Section 6.5.2.6 of the Technical Specification states, "The minimum quorum of the SRC necessary for the performance of the SRC review and audit functions of these Technical Specifications shall consist of... " and Section 5.2.7.d states, "The SRC shall review proposed changes to Teci ical Specifications on this Operating License."
Contrary to the above, a quorum was not formed to approve a change to the Technical Specifications in that on August 31, 1978 the SRC neabers t!ere contacted individually by the SRC Secretary for approval to change section 3/4 6-5.
This is an infraction.
