Appendix A NOTICE OF VIOLATION Taylor University License No. 13-04004-02 License No. SNM-700 Based on the inspection conducted on November 16, 1978, it appears that certain of your activities were in noncompliance with NRC requirements, as noted belott.
Items A.I. and S. are infractions, and item A.2.
is a deficiency.

License No. 13-04004-02 1.
Condition 8.U. of your license linits the amount of cesium-137, as a sealed source, that you may possess at any one time to 3 mil 11 curies.
Contrary to this requireaent, on the day of the inspection, you possessed a cesium-137 sealed source of approximately 4 milli-curies 2.
10 CFr. 30.51 requires that you keep records showing the receipt, transfer, and dispos 91 of byproduct material.
Contrary to this requirement, as of November 16, 1978, you failed to naintain receipt records for the bypreiuct material you received from RCA in about 1970.
B.
License No. SNM-700 Condition 12. of your license requires that you test your plutonium sealed source for leakage at intervals not to exceed six (6) months.
Contrary to this requirement, you failed to test your plutonium sealed source for leakage from August 12, 1975 to August 26, 197o and from August 26, 1976 to May 24, 1977, periods in excess of six months.
The latest leak test was performed on November 15, 1978. The inspector was informed that a leak test was performed sometiae between May 24, 1977 and November 15, 1978, but no record of such test could be produced to verify the statement.
