Inspection Report 50-285/78-14 Dated November 20, 1978 In reply to the above-referenced inspection report, the Omaha Public Pove r District herein submits comments to indicate the status of the reported items of noncompliance.

Infraction Section 17.5 of your NRC approved Quality Assurance Program and Criterion V of Appendix B to 10 CFR 50, require procedures to prescribe the appli-cable acceptance criteria.

Contrary to the above, the surveillance test procedure, Surveillance of Grinnell Hydraulic Shock Suppressor Reservoir Oil Levels in Containment (ST-HSS-5), does not contain acceptance criteria for the reservoir oil levels.

Response

(1) Corrective steps which have been taken and the results achieved.

Appropriate acceptance criteria have been developed as indicated in item (2) below. In addition, all previous reservoir oil level data gathered per Surveillance Test ST-HSS-5 during the previous fuel cycle was reviewed. No abnormal oil levels were identified, thus indicating that all the shock suppressors in question were operable during the past year.

Subsequent to plant shutdown on October 15, 1978, for refueling, the reservoir oil levels of the subject shock suppressors were again inspected. Applying the newly defined acceptance criteria, this inspection indicated that all the subject shock suppressors were operable.

In accordance with Standing Order G-30, Procedure Change No. 3873 was prepared to add acceptance criteria for the reservoir oil levels to Surveillance Test ST-HSS-5 (3) The date when full compliance vill be achieved.

Procedure Change No. 3873 has been completed and the subject shock suppressors vill be examined accordingly during future inspections.

The subject shock suppressors are operable; thus, the District is presently in full compliance.

Infraction Technical Specification 3.1h(1) requires that all hydraulic shock suppressors of specified categories be visually inspected at given i.spection intervals determined by the results of the previous inspections.

Contrary to the above, following the reporting of fifteen inoperable Grinnell shock suppressors on November 22, 1977, performance of Sur-veillance Test ST-HSS-4 (Surveillance of Grinnell Hydraulic Shock Suppressors Reservoir Oil Levels in Containment) on dates January 6, 1978, February 10, 1978, March 31, 1978, May 17, 1978, and September 8, 1978, did not include a visual inspection of the shock suppressors.

Response

(1) Corrective steps which have been taken and the results achieved.

All of the subject shock suppressors were visually inspecte'd during plant startup following the 1977 Refueling Outage (December, 1977).

The visual inspection performed at that time indicated that the shock suppressors were operable.

Subsequent to plant shutdown on October 15, 1978, for refueling, all of the subject shock suppressors were again visually inspected.

The results of this inspection indicated that all the subject shock suppressors were operable.

(2) Corrective steps which vill be taken to avoid further noncompliance.

In accordance with Standing Order G-30, Procedure Change No. 3873 was prepared to include a visual inspection of the subject shock suppressors on Surveillance Test ST-HSS-5
