OCT 2 61979 APPENDIX A NOTICE OF VIOLAT10N Duke Power Company License No. DPR-38 Oconee Nuclear Station DPR-47 DPR-55 Based on the NRC inspection September 4-28, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.

These items have been categorized as described in correspon-dence to you dated December 31, 1974.

A.

As required by 10 CFR 50, Appendix B, Criterion V, Instructione, Proce-dures and Drawings, " Activities affecting quality shall be prescribed by documented instructions

. and shall be accomplished in accordance with these instructions

." The accepted Quality Assurance Program, DUKE-1, states in part, " Procurement of materials, parts, and components

... con-trolled.. to assure that the safety.. of the station is not compromised."

Also, Administrative Policy Manual 4.5 that implements the Quality Assurance Program states, "Each purchase specification shall be prepared by a qualified individual who shall assure that all applicable requirements are included, and shall be reviewed to assure accuracy of content."

Contrary to the above, Purchase Oi Tr No. C 60920, dated July 12, 1976, was prepared and issued for material that did not meet the requirements for use at Oconee Nuclear Station.

Specifically 0-Rings for the reactor vessel head had 16 slots instead of 12, and the 0-Ring diameter was 0.a inches instead of 0.455 inches.

This is an infraction and applies to the three Oconee Units.

B.

Technical Specification 6.4.1(e) requires preventive or corrective main-tenance to be in accordance with approved written procedures.

Contrary to the above, the reactor vessel head 0-Rings for Unit I were not installed in accordance with procedure MP/0/A/1150/09, on September 29, 1978.

This is an inf raction and applies to Oconee Unit 1.
