TOLEDO EDISON COMPANI DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-80-02 DATE OF EVENT: January 3, 1980 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Auxiliary Feed Pump 1-1 inoperable Conditions Prior to Occurrence: The unit was in Mode 3, with Power (MRT) = 0, and Load (Gross MWE) = 0.

Description of Occurrence: At 2230 hours0.0258 days <br />0.619 hours <br />0.00369 weeks <br />8.48515e-4 months <br /> on January 3, 1980 during performance of Auxiliary Feed Pump 1-1 Surveillance Test ST 5071.01, problems were encountered with the turbine governor for Auxiliary Feed Pump 1-1.

It stuck at 3400 RPM.

Due to inability to raise turbine speed to correspond with the high speed stop settings of.the governor, the pump was declared inoperable.

This placed the unit in the Action Statement of Technical Specification 3.7.1.2 which requires that with one pump inoperable, the inoperable pump must be repaired within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or the unit must be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />. Tech-nical Specification 3.7.1.2 requires operability of both Auxiliary Feed Pumps while in Modes 1, 2, and 3.

Designation of Apparent Cause of Occurrence: The cause of this occurrence was determined by station personnel and a Woodward Governor representative to be failure to readjust the slip clutch mechanism to compensate for normal wear. The slip clutch is installed between the governor manual speed adjustment shaf t and a remotely controlled speed changer motor, and protects the speed changer motor from overheating when the governor's mechanical high speed stop prevents further manual speed adjustment shaf t rotation.

In addition, a burr was present on the high speed stop pin which caused the pin to hang up prior to reaching the high speed stop position. This burr added further resistance to the slip clutch, and the clutch slipped prematurely.

Analysis of Occurrence: There was no danger to the health and safety of the public or to station personnel. Auxiliary Feed Pump 1-2 was operable during the inopera-bility of Auxiliary Feed Pump 1-1.

Corrective Action

On January 4, 1980 under Maintenance Work Order 80-1224 the slip clutch was properly tensioned, and the small burr on the high speed stop pin was removed. The Auxiliary Feed Pump 1-1 Test ST 5071.01 was successfully completed, and the system declared operable at 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> on January 5, 1980. This removed the unit from the action statement.

A preventive maintenance instruction MI-M-95 was written and a preventive maintenance work order will be initiated to ensure the slip clutch for the speed changer motor is periodically inspected and properly adjusted.
