Appendix A NOTICE OF VIOLATION Consumers Power Company Docket No. 50-329 Docket No. 50-330 Based on the results of an NRC inspection conducted on August 5-September 28, 1979, it appears that certain of your activities were not conducted in full compliance with NRC requirements as noted below. This item is an infraction.
10 CFR 50, Appendix B Criterion V, states, in part, that, " Activities affecting quality shall be prescribed by documented instructions, procedures,... and shall be accomplished in accordance with these instructions, procedures,..."
Bechtel Engineering Department Procedure BEDP-4.63 states in part, "The Supplier shall be required to submit deviation requests to the Bechtel Project Engineer with a copy to the Bechtel Shop Inspector within five (5) wo'.'-ing days after detection."
Contrary to the above all four auxiliary feed water pumps were shipped to the site and installed with oil coolers which were nonconforming with the require-ments of ASME Section III and without the Supplier being required to submit a Supplier Deviation Disposition Request. The cognizant field engineer was knowledgeable of the pumps being shipped to the job site without the correct coolers.
