PROPOSED IE INFORMATION NOTICE: PRESSURE SWITCH FAILURE - OHI 1 On July 14, 1979, the OHI 1 (Kansai Electric Company's 1175 MWE PWR) reactor was snutdoan due to the failure of a generating system water pressure gauge supplied by Westinghouse.
Specifically, the pressure gauge f ailed as a result of damage to the copper alloy bourdon tube, which in tarn opened a safety valve, causing a loss of pressure and subse:uent ECCS actuation.
The bourdon tube was incorrectly labeled as being.anufactured of s tainless steel.
In addition to the failed gauge, i t was foun: that other similar pressure gauges were improperly labele: at the time of their installation.
