SUMMARY

OF KEY EENTS WEEK ENDING DECEMBER 14, 1979 North Anna, Unit 1 On Friday, November 30,1979, VEPC0 and Westinghouse met with the NRC staff to further discuss the discovery of cracks in steam generator tubes.

The results of the eddy current testing (ECT) for Steam Generators A, B, and C show that for the 888 tubes inspected, 35% had ECT indications of support plate corrosion or cracking. The staff requested VEPC0 and Westinghouse to document the results of the North Anna 1 inservice.inspec-tion and supply the basis for proposed corrective actions as soon as possible for NRC staff review prior to closing up the steam generators.

Quad Cities On December 5,1979, the resident inspector called to say that two core spray suction valves for Unit 1, whose normal alignment is open, were found closed. The valves automatically open on ECCS signal, so no safety oroblem was involved. However, the resident inspector, on December 6,1979, said that authorities apparently suspect sabotage.

Point Beach 1 On December 11, 1979, Point Beach Unit 1 experienced a sudden step increase in primary-to-secondary leakage (50 to 250 gallong per day). This event followed by eleven days the issuance of a confirmatory order which imposed additional operating restrictions with respect to steam generator tube integrity.

NFS West Valley Site Personnel from CA0 met with several members of the NRC staff to conduct an entrance conference to discuss the initiation of a GA0 review of Nuclear Fuel Services, Inc. (NFS), West Valley facility. GA0's objective is to identify remaining problems at the facility and to detennine the status of actions being taken by NRC, DOE and EPA to resolve these problems.

In addition to following up on their previous recommended acticns, GA0 will try to show the relevance of the NFS-West Valley problems to current national issues demanding resolution, such as decommissioning and high level waste storage.

Procosed Reouirements for the Protection of Transient Shioments A working staff meeting was held on December 13, 1979 at the Department of State to consider legal aspects and best approaches to amend Panama Canal regulations to protect transient shipments of formula quantities of SSNM through the canal.

The meeting should provide a coordinated interagency staff cosition en how to proceed to assure protection of transient shipments through the Panama Canal.

....,g; DIVISION OF RULES AND RECORDS OFFICE OF ADMINISTRATION The Executive Director of Operations has published a proposed amendment to the NRC Notice of Systems of Records to add a new sys' tem, IE Household Move Survey, NRC-35.

The purpose of the system is to

' enable the NRC to obtain data on actual costs of household moves of employees (such as resident inspectors) whose duty station is transferred, in order to compare their costs with the amounts reimbursed to them by the Government.

results of the eddy current testing (ECT) for Steam Generators A, B, and C are presented below:

A total of 888 tubes were inspected for SG's A, B, and C.

Testing w'as con-ducted for 400 khz,100 khz, and 71/2 khz for all three SG's.

Two cracks were found tangent to the U Bend in two tubes of SG C (Row 1 - columns 51 and Also based primarily on 71/2 khz testing, indications from ECTs on 60).

306 tubes indicated the presence of support plate corrosion (or cracking).

Thus, for the 888 tubes inspected, 35% had ECT indications of support plate corrosion or cracking. Westinghouse stated that corrosion in the annulus between the plate and the tube is full blown. Westinghouse also stated the support plate corrosion occurs before the cracking.

Westinghouse believes that the support plate damage observed was caused by the inadvertent dumping of 200 to 300 pounds of resins from the polishers of the full flow demineralizers into SG's.

Sulfonic acid at high temperature produced sulfuric acid which attacked carbon steel.

(Westinghouse said a similar problem with a resin bed occurred at a Belgium reactor.)

The staff requested VEPCO and Westinghouse to document the results of the North Anna 1 inse.vice inspection and supply the basis for proposed corrective actions as soon as possible for NRC staff review prior to closing up the steam VEPCO and Westinghouse have been requested to present the technical generators.

VEPC0 will plug all tubes in the innermost row (row 1) in all three steam generators.

VEPC0 has also committed to installing inspection ports, as recuested by the NRC staff, in the three SGs during the second refueling outage.

Similar ports have been installed on SGs at other operating Westinghouse plants. The NRC staff is currently reviewing the VEPC0 report, submitted December 10, which documents the results of its recent inservice inspection of the North Anna i steam generators.

The staff will prepare a safety evaluation on this matter before the plant returns to power.

i Ginna During an unscheduled shutdown to repair B steam generator tube leaks and while performing dye penetrant In-Service Inspections, linear indications were observed in the'4-inch diameter pressurizer nozzle safe end. The safe end is welded to a pipe to the T cross piece for the two Power Operated Relief Valves (PORVs).

Additional dye penetrant tests, after polishing the crack area, showed the indications to be larger than first measured. The family of small indications fall within a 1/8 to 3/16 inch circumferential band width between the 3 and 10:30 o' clock locations. The band of flaws is in the safe end apprdximately 1/8 inch from the weld fusion zone. The indications have not been detected by using Xray or ASME Section XI ISI ultrasonic techniques. Westinghouse and Southwest Research are preparing to re-examine the affected area using moreThe definitive Xray and UT methods to determine the depth of the indications.

staff will continue to evaluate the new ISI data as it becomes available to determine the effect of the flaws in the schedule 160 piping integrity, the necessity for repairs, and the impact on similar plants.

Pilcrim Unit No.1 While in the process of storing new fuel assemblies in the spent fuel gool, a spent fuel assembly was inadvertently raised to a level (under water, which caused the refueling floor radiation monitors to alarm ('setpoint 100 mr/hr). This occurred at 5:45 a.m. on December 11, 1979. Apparently a rope is used to disengage a spring loaded clip in the grappling tool. The forces applied in the disengaging process were sufficient to rotate the tool such that it hooked an adjoining spent fuel assembly. The three people involved in the evolution received approximately 30 mr, before the assembly was restored to its original position.

Reportedly there is a major network news team on site.

Salem i On December 7,1979, Salem Unit No.1 was brought to operational Mode 5 (Cold Shutdown) from cperational Mode 2 (Startup) when ongoing analyses to implement IE Bulletin 79-07 revealed seismically unqualified pipe succort in the three diesel ger.erator rooms. On December 10, 1979 PSE&G notified the Project Manager that the Mode 5 period would be extended to approximately 22, 1979 to pennit (1) further analyses of pipe supports, (2) modifications December of the containment su p, and (3) ccmpletion of Lessons Learned requiremants.

Trojan The Atomic Safety and Licensing Board (Trojan Control Building Proceeding) has ordered a two day special hearing to be held in Portland, Oregon on December 28 and 29, 1979. TTie heari.ng wiil consider the recent wall / piping support reaction problem as well as unreso ved issues related tc the proposed control building modification, including the issuance date of the NRC staff's SER and a discussion of how long interim operation of Trojan should be per-mitted.

Resumption of Trojan operation is currently prevented pending further order of the Licensing Board (November 30, 1979 Board Order).

The Commission was briefed on the " deep crevice cracking" problem at this unit on November 28, and was briefed on this event on Deccmber 12.

The unit has been shut down for inspection.

Clinton Station Illinois Power Company tendered an operating license application on December 5,1979. The Clinton Station will have two MWe BWR-6 Mark III Containments with the first unit ready for fuel loading in 1983.

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending December 14, 1979 Radon Releases from Solution Mining It is forecast that during the next 20 years solution mining may increase until it provides about 10% of the uranium produced in the U.S.

Recent data indicate that solution mining dissolves radon as well as uranium from the underground ore body. When the solution is pumped to the surface, radon escapes as the pressure is released in the surge tank. The radon releases per tm of uranium produced appear to be about half those of open pit mining.

Proposed Revisions to 10 CFR Part 71 Representatives of NRC attended a meeting at the request of DOE to discuss DOE draft comments on the proposed D0T/NRC rulemaking.

DOE objects to (1) the proposed upgrading of packaging requiremente for the shipment of LSA materials, (2) inconsistencies and duplications in DOT and NRC proposed rulemaking, and (3) the inconsistency of the proposed domestic regulations and those recommended by the IAEA.

The comment period closes on December 17, 1979, at which time all comments received will be considered by the staff.

Meeting with FDA's Bureau of Drugs NMSS Representatives attended a meeting with FDA's Bureau of Drugs. This was the secenc meeting to define the paralled roles each occupies in the assurance of safety and efficacy of radioactive drugs and the regulation of nuclear pharmacies. Many specific items were discussed and several issues clarified by each agency. Working groups were appointed to follow up on several items and to arrange for another meeting in the near future.

NFS West Vallev Site Perunnel from the General Accounting Office (GAO) met with several members of the NRC staff to conduct an entrance conference to discuss the initiation of a GA0 review of Nuclear Fuel Services, Inc. (NFS), West Valley facility.

GA0's objective is to identify remaining problems at the facility and to determine the status of actions being taken by NRC, DOE, and EPA to resolve these problems.

GAO will follow-up on their previous recommended actions listed in tneir March 1977 report, " Issues Related to the Closing of the Nuclear Fuel Services, Incorporated, Reprocessing Plant at West Valley, New York," (EMD-77-27).

In addition, GA0 will try to show the relevance of the NFS-West Valley problems to current national issues demanding resolution, such as decomissioning and high level waste storage. The staff informed GAO that their primary NRC contacts on the status of actions related to NFS-West Valley should be Fuel Cycle licensing personnel.

US/IAEA Safeguards Acreement On Tuesday, December 11, 1979, the Senate Foreign Relations Committee held a meeting to permit Senator McClure to present testimony related to the US/IAEA Safeguards Agreement now being considered by the Senate as a treaty.

Senator McClure indicated that he now recommends that the Senate give its consent to the treaty in the near future, with the reservation that the President does not put the agreement into force until legislation providing for domestic implementation has been enacted by Congress.

Follow-up meetings between the staff and representatives of various other U.S. agencies are being set up to consider all of Senator McClure's concerns in detail.

0FFICE OF INTERNATIONAL PROGRAMS WEEK ENDING DECEMBER 14, 1979 INTERNATIONAL COOPERATION Meeting with Members of Embassy of Japan On Tuesday, December 3, representatives of IP and SD met with First Secretary Miyabayashi and Mr. Shirao of the Embassy of Japan to discuss recent NRC activities associated with emergency planning and siting.

The Government of Japan has displayed a keen and continuing interest in these two topics because of the potential impact that new NRC emergency planning and siting regulations may have in densely pop'21ated Japan.

Canadian Follow-Up IP, in follow up to the November 27-28 visit of the ll-member Canadian delegation headed by AECB President Jon Jennekens, is canvassing the involved NRC staff to identify which subjects raised during their discussions merit further study and/or possible trips to Canada.

Emergency planning and operating experience have already been identified. The Canadians have also recommended a meeting in Toronto (and a one-day visit to the Bruce Nuclear Power Development) in April or May 1980 to allow them to better e:g'.ain their differences in implementing what appears to be a commonality of safety philosophy EXPORT / IMPORT AND INTERNATIONAL SAFEGUARDS Agreements for Cocoeration During the first week of December, IP received from State several draft documents pertaining to ongoing renegotiations of agreements for cooperation.

A draft Protocol amending the agreement with Canada was received and is now under review by the staff.

An updated draft of proposed modifications to the U.S.-

Taiwan agreement has also been received and is being reviewed.

A Commission action paper on the U.S.-Indonesia agreement has been drafted and will be forwarded for final Commission concurrence as soon as interoffice coordination is completed.

Nuclear Excort Referral List On December 10, IP represented NRC at an interagency meeting at the Commerce Department to discuss the preparation of the revised and unclassified Commerce Department " Nuclear Export Referrai List." This list, which is being prepared pursuant to the Export Administration Act and Section 309(c) of the Nuclear Non-Proliferation Act, will contain those items under Commerce Department export control authority "which could be, if used for purposes other than those for which the export is intended, of significance for nuclear explosive purposes." Export of items on the list will be subject to interagency review for nonproliferation curposes.

OFFICE OF STATE PROGRAMS ITEMS OF INTEREST WEEK ENDING DECEMBER 14, 1979 Emercency Precaredness On November 28, 1979, Tom Elsasser, Region I SLO attended a meeting in Region IV with State Liaison Officers.

He discussed two separate topics:

The role of the Office of State Programs in Emergency Preparedness and the Public Informational Aspects of the Accident at Three Mile Island.

A radiological emergency response coordina tors course was conducted for Florida personnel in Wildwood, Florida.

The course teaches the basics of dose proj ection,. accident assessment, and protective action decision making for State and local perscnnel.

Requests have been made to NRC for at least 5 or 6 courses for other parts of the country in FY 1980.

On December 12, 1979, Dick Van Niel met with FEMA and Regional Advisory Committee Region V personnel to discuss the Wisconsin Radiological Emergency Response Plan.

The State will be notified of additional areas needing improvement.

The Region V Regional Advisory Committee Chairman, Tom Essig, met with Mr. Erie Jones of Illinois to discuss the Illinois Radiological Emergency Response Plan, due for submittal for review next month.
