99900391/79-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on October 1-5, 1979, it appears that certain of your activities were not conoucted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR 50 and paragraph NCA-4134.5 of Section III of the ASME Code states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, proceaures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for dt.termining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

Paragraph 6.2.1. of the ASME accepted Quality Assurance Manual states in part

" Drawing Prints and Quality Control Plans used for machining are controlled documents issued by Engineering in accordance with Section 4.

Paragraph 3.4.1. of the ASME accepted Quality Assurance Manual states "All changes made to Design Documents after release are controlled with Engineering Change Notices."

Paragraph 28.1 of Quality Assurance Procedure QAP 28 Revision 1 states in part " items manufactured by MCCI are subjected to First Piece Inspection for each sequence of Operation on the manufacturing procedure (Operation Sheet)."

Contrary to the above, Production changed the tolerances on two dimensions on Drawing 188895-9001 f rom + 0.010 to +0.005 to control dimension on subsequent operation without an Engineering Change Notice and/or Operation Sheet.

It was determined by review of other drawing, issued to Production, and interviews with personnel that this was a single isolated occurrence. The quality assurance man-ager verified that the tolerance change was correct, and to prevent recurrence issued an interoffice memo to Production, dated October 4, 1979, stating that the practice of altering controlled drawings was unacceptable.

The deviation is closed and no answer is required.
