Appendix A NOTICE OF VIOLATION Northeast Nuclear Energy Docket No. 50-245 Company Based on the inspec 4on conducted on June 25-29, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.

Item I is an infraction and item 2 is a deficiency.

Technical Specifications, Section 4.2, Table 4.2.1, requires a functional test to be performed on the ventilation exhaust duct and refueling floor radiat4on monitors, once every 3 months. Technical Specifications, Section 1, Item F defines " instrument functional test" to mean the injection of a simulated signal into the instrument primary sensor to verify the proper instrument channel response,.

alarms, and/or initiating action.

Cnntrary to the above, the licensee's functional test of the above uescribed equipment consisted of injecting an electrical signal into the measurement channel and not testing the primary sensor as required.

Technical Specifications, Section 6.8.1 requires that procedures be established, implemented and maintained for surveillance activities.

The licensee's administrative control procedure, ACP-QA-9.02 requires the licensee to review the surveillance test data sheets for compli-ance with the established acceptance criteria and to write on the data sheet the basis for accepting those items not within the estab-lished acceptance criteria.

Contrary to the above, the inspector found eight surveillance tests where the data on the data sheets did not meet the established acceptance criteria and were not properly reviewed as required by the licensee's administration procedures. These data sheets were:

SP412C, dated August 8, 1978 and September 5, 1978:

SP412L, dated December 12, 1978:

SP406T, dated March 24, 1978:

SP621.10, dated April 10, 1978, May 12, 1978 and September 15, 1978: SP623.19, dated May 12, 1978.
