One of the activities under your license appears to be in violation of AEC requirements. This violation is considered to be of Category III severity.

Title 10, Code of Federal Regulations, Part 50, Appendix B, Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings,
of a type appropriate to the circumstances and shall be accomplished C
in accordance with 'these instructions procedures, or drawings including changes thereto, which prescribe all activities affecting quality".
Contrary to the above, the Test Manning Assignment Docu=ent for the Reactor Coolant System Hydrostatic Test, which was approved and e.
issued on October 26, 1973, did not specify the stations required to be manned during this test, as required by the Test Manual and Test Instruction No. 3.
Additionally, the approved Test Plan for

October 29-November 5,1973 did not identify the Test Manning Assignment Document for the Reactor Coolant System Hydrostatic Test, as required by Test Instruction No. 2.
During the inspection and prior to. the performance of the Reactor S
Coolant System Hydrostatic Test, the Test Manning Assignment Document and the Test Plan were revised to conform to applicable requirements of the Test Manual and Test Instructions No. 2 and 3.
