ENCLOSURE 1 DESCRIPTION OF VIOLATIONS I
Metropolitan Edison Company Docket No. 50-289 License No. DPR-50 i
- Certain activities under your License appear to be Category II violations of AEC requirements.
These violations have been reported to the AEC as
~
Abnormal Occurences and follow up action has been taken.
1.
Technical Specification, Paragraph 3.1.9.1.b, provides the following restriction for low power physics testing:
"Above 1800 psig nuclear overpower trip shall be set at less than 5.0 percent..."
Contrary to the above, on June 6, 1974, the gain setting of the Channel i
B nuclear overpouer trip was found to be improperly set, resulting in a trip setpoint of 20%, conservatively computed. This gain setting was i,serted prior to the reactor's initial criticality on June 5, 1974 9}, -
and resulted in the criticality evolution being performed with the incorrect trip setpoint.

Technical Specification, Paragraph 3.6.1 states:
" Containment in-

tegrity, as defined in Section 1.7, shall be maintained whenever all three of the following conditions exist:
a.
Reactor coolcnt pressure is 300 psig or gr,ea.ter.
b.
Reactor coolant temperature is 200 F or g? eater.

c.
Nuclear fuel is in the core."
I Contrary to the above, on June 13, 1974 with the Reactor Coolant System at a temperature of 5200F and a pressure of 2155 psig and fuel in the core, both doors of the Reactor Building Personnel Hatch were opened si=ultaneously.
This resulted in the degradation of the. containment integrity.
We note these matters were reported to Regulatory as indicated above.
Your corrective measures were reviewed during this inspection.
We have no further' questions on these matters at this time.
