i APPENDIX A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on September 21-23, 1977, it appears that two of your activities were not conducted in full compliance with conditions of your NRC Facility License No. DPR-50 as indicated below.
A.
10 CFR Part 50, Appenjix B, Criterion XVI states:
" Measures shall be established to assure that conditions adverse to quality, such l
as... malfunctions...are promptly identified and corrected."
The licensee subscribed to 10 CFR Part 50, Appendix B, in Paragraph 20 of Appendix 1A of his Final Safety Analysis Report.
In the same paragraph, it states:
...nonconfonnance such as equipment malfunction...at the station are identified and resolved."

I Contrary to the above, the ventilation fan B in the battery rooms was inoperable.

10 CFR Part 50, Appendix B, Criterion V states:
" Activities affect-j ing quality shall be prescribed by documented instructions...and shall be accomplished in accordance with these instructions...."
The licensee subscribed to 10 CFR Part o0, Appendix B, in Paragraph 10 of Appendix 1A of his Final Safety Analysis Report.
In the same paragraph, it states:
... instructions... associated with the administration...of structures...are... implemented...."
The door to the cable spreading room is posted with a "No Smoking" sign.
Contrary to the above numerous cigarette butts were found on the floor of the cable spreading room.
This item is an infraction.
