i Three Mile Island Nuclear Statien, Unit 1 Operating License No. DPR-50 Docket No. 50-289 Inspection Report No. 77-29 This Letter and the attach =ent are in response to your inspection letter of October 20, 1977, concerning Mr. L. Gage's inspection of TMI-1 and the resultant finding of two apparent infractions.

i Infraction A 10 CFR Part 50, Appendix 3, Criterien XVI states: " Measures shall be established to assure that conditiens adverse to quality, such as...=alfunctions... are prc=ptly identified and corrected."
The licensee subscribed to 10 CFR Part 50, Appendix 3, in Paragraph 20 of Appendix 1A of his Final Safety Analysis Report.
In the same paragraph, it states:
"...nonconfor=ance such as equipment
=alfunction...at the station are identified and resolved."
Contrary to the above, the ventilation fan B in the battery roc =s was inopere.ble.
Response to Infraction a When notified that there was no ventilation flev in the battery rec =s, operations personnel checked the system. No specific cause for the no flow condition was found. Flow was restored when ventilation cyas shifted to the alternate side. The A syste= (AH E ITA, AH E 95A) d the B syste= ( AH E 173, AH E 953) were functionally tested.
Flov was obtainable using either system. However, during the functional test it was noted that the AH E ITB fan did not start on the first atte=pt. This failure to start may have been caused by not maintaining the switch in the start position until the da=pers fully opened.
Subsequent starts on the 3 fan were satisfactory.
Corrective action to prevent recurrence vill be to check ventilatien flow in the battery roc =s each shift. Full ec=pliance was achieved on 11/0T/77 Infraction 3 10 CFR Part 50, Appendix 3, Criterion V states:
" Activities affecting quality shall be prescribed by docu=ented instructions...and shall be accc=plished in accordance with these instructions..."

Contrary to the above, numerous cigarette butta vere found en the ficor of the cable spreading rocs.
Restense to Infraction 3 I==ediate action taken included a thorough cleaning of the cable spreading roca and reiteration to Maintenance and Operations Department Supervisors of the necessity to enforce "No S=oking" areas. Additional "No Sncking" signs have been placed in nu=erous locations throughout the interior of the cable spreading rocs to serve as a further reminder to those verking in the room.
Supervisors are nov required to tour the cable spreading rocs at least daily and to take i==ediate corrective actica on any discrepancies noted.
In addition, a cigarette receptacle has been purchased and installed at the entrance of the cable spreading rces to facilitate disposal of cigarettes prior to entering the rocm. Full ec=pliance has been achieved.
