License No. DPR-50 APPENDIX A NOTICE OF VIOLATION Based on the results of an NRC inspection conducted on August 30 through September 2,1977, it appears that one of your activities was not conducted in full compliance with the conditions of your NRC Facility License No. DPR-50 as indicated below.
This item is categorized as an infraction.
10 CFR 50, Appendix B, Criterion VI, states in part, " Measures shall be established to control the issuance of documents, such as... procedures... including changes thereto, which prescribe all activities affecting quality.
These measures shall assure that documents, including changes...are distributed to and used at the location where the prescribed activity is performed..."
Contrary to the above, measures were not established to control the issuance of documents, in that during the period of January 21, 1977 through August 18, 1977, one surveillance activity of the eight surveillance activities inspected and three operations C2 activities of the six operations activities inspected were accomp-lished using procedures that were superseded by revisions to these procedures.
