Docket No. 50-289 License No. DPR-50 Inspection No. 77-27 RESPONSE TO DESCRIPTION OF /JpARENm VIOLATION Apparent Violation 10 CFR 50, Appendix B, Criterion VI, states in part. " Measures shall be i established to control the issuance of documents, such as ... procedures

. .. including changes thereto, which prescribe all activities affecting quality. Tlese measures shall assure that documents, including changes

Contrary to the above, measures were not established to control the issuance f of documents, in that during the period of January 21, 1977 through August 18 3 l 1977, one surveillance activity of the eight surveillance activities inspected and three operations activities of the six operations activities inspected were accomplished using procedures that vere superseded by revisions to these ,

y procedures.

from controlled copies for use during the refueling outage startup. However, ,

the procedures were not used immediately because of delays in plant startup. l' The procedures were revised during the delays in plant startup, and the copies

. which had been xe' sxed were not revised. A me=o was distributed on October 11, 1977, requiring t'aat copies of procedures be verified as current immediately before their use.

The use of outdated forms in the perfor=ance of SP 1301-5.8 was due to keeping i data for=s in a separate file. These forms were not updated when the procedure {

vas revised. These blank form files have been eliminated, and in the future, -

data sheets vill be xeroxed directly from the controlled copies of the procedure i to prevent reoccurrences.
