=inistrative policies shall be established, imple=ented and =aintained..."

Contrary to the above, at approxinately 5:30 a.=.

on April 17, 1977, valve LR-V7 was found shut when required by step 6.h.e. (h) of Surveillance Procedure 1303-6.1, " Reactor Building Integrated Leak Rate Test", Revision 3, dated April 9,1977, to be open, providing an i=prcper barrier to con-tain=ent leakage-Response to Arparent Violatien i

The valve (LR-V7) vas positioned properly prior to taking the acceptable leak rate data therefore having no effect en the final results.

The step in the procedure which was violated, required the positioning of I

two (2) valves. However, only one (1) valve vis positicaed before going on to the next step. The procedure vill be changed to require initialing each step when it is perfor=ed. This will assure a correct operation and should,

i preclude a repetition of this occurrence.

O This procedure revision vill be in effect prior to the next perfor=ance of this procedure.
