REACTOR TRIP DUE TO THE LOSS 07 A FEEDWATER PUMP CAUSED BY PERSONNEL ERROR
~~
A reactor trip from 90% power occurred at 1900 hours0.022 days
0.528 hours
0.00314 weeks
7.2295e-4 months
on July 24, 1979, due to the loss of one of the two operating feedwater pumps caused by personnel error.
The error occurred during operations to return the 2C Steam Driven Feedwater pump to service following maintenance.
The operating engineer inadvertently backed down the hand speed changer on the operating 2B Steam Driven Feedwater pump instead of the 2C Steam Driven
~
Feedwater pump, thereby causing the reduction of feedwater flow and sub-sequent plant trip.
This adjustment error was made locally, and was supposed to have been made on the 2C pump bec.mse of difficulties encountered in the control roon in getting the pump latched.
Plant systems functioned normally and there were no abnormalities associated with the event.
Plant startup is in progress and the unit is expected to be returned to service today.
The resident inspector will perform inspection followup of this event giving close attention to the cause of the personnel error and the licensee's actions to prevent a recurrence.
A press release is not planned by the licensee nor RIII.
The State of Illinois is being notified.
RIII was notified of this matter by the Resident Ins m tor at 9:00 a.m.
on July 25, 1979.
