The information presented is as i

SUBJECT:

LOST SEALED SOURCES Five sealed sources used for oil well logging were lost overboard in the Gulf of Fexico approximately six-miles from Grand Island, Louisiana.

The sources, two (2) 11s Curies Cs-137, one (1) 16 Curies Am-Be, and two (2) is Curies Am-Be, were in shielded containers within a tool box which was loaded on a pallet on a boat.

The boat was in transit from an offshore platform to shore when the pallet was lost overboard and sank during a tropical storm on July 24, 1979.

The licensee has notified the State of Louisiana.

There has been no media coverage nor is any expected.

Region IV (Arlington, Texas) received notification of the loss by telephone f rom the licensee's Radiation Safety Officer at 3:40 p.m. on July 25, 1979.
