January 26, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-13 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance.
The information presented is as initially received without verification or evaluation f
and is basically all that is known by IE staff on this date.

Subject:
MISSING PACKAGE CONTAINING 28 MILLICURIES OF 10 DINE-131 A licensee representative reported that a package containing 28 millicuries of iodine-131 in two 14 millicurie therapy capsules is missing.
The missing package was delivered, signed for by a hospital employee and placed in a locked closet shortly after 3:30 a.m. on January 25 for storage.
When Nuclear Medicine personnel checked the closet at 9:30 a.m. on January 25, they could not find the package. A search of the hospital did not locate the package.
The shipper, Mallinckrodt Nuclear, St. Louis, Missouri, has confirmed the isotope and activity.
The carrier, United States Priority Transport Company, Huntington Station, New York, has confirmed that their driver possesses a signed receipt for the package.
The iodine-131 capsules do not present a threat to individucir ao long as the cepsules remain within the shielded container.
Ingestion of the capsules would deliver a medically significant dose to an individual's thyroid.
A Region I inspector has been dispatched to the licensee's facility and is aiding the licensee in the search for the missing package.
The Commonwealth of Pennsylvania has been informed.
A press release is being considered by Region I.
NRC, Region I, (Philadelphia) received notification of this occurrence by phone call from the Radiation Safety Officer at Bryn Mawr Hospital at 11:45 a.m. on January 26.
