PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-U5

This preliminary notification constitutes EARLY notice of event of P_0SSIBLE safety or public interest significance.

The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Facility:

Iowa Electric Light and Power Company Duane Arnold (DN 50-331) Palo, IA

Subject:

ALLEGED LICENSEE ACCEPTANCE OF REJECTABLE WELDS A nondestructive testing company (Peabody Testing) Level II inspector has raised questions relative to adequacy of a total of nine welds on seven of the eight recirculation inlet nozzle repairs.

The allegations were made orally to Region III (Chicago) inspectors.

The major concern of these inspectors was that pressure from the licensee forced the acceptance of welds which were, in their opinion, clearly rejectable.

This concern was documented by the three inspectors in a letter that they transmitted to their management about Dec.cmber 20, 1978.

Since that time, the inspectors have been relieved of their interpreting responsibilities, and their activities have been limited to making radiographs.

The alleger has also stated that in reviewing the entire NDE documented package for completeness, he found several instances in which the original interpretation document had either been replaced or changed.

The alleger's concerns include radiographic test records of welds previously questioned by Region III inspectors which the iicensee had stated they were going to provide additional information to justify acceptance of the welds.

Region III inspectors have reviewed this additional data and continue to question its adequacy.

Region III inspectors will continue investigation of the specific areas of the allegations this week.

The allegation was made to the Region III inspector early on January 11, 1979.

This information is current as of 8:00 a.m., January 15, 1979.

There has been media interest in the general subject of repair of these wel ds.

It is not known whether the allegation will attract media interest.
