This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest si nificance.
The information presented is aq initially received without verification or evaluation and is basically all that is known by IE staff on this date.
Facility:
Consumers Power Company Palisades Nuclear Generating Plant, Covert, MI (DN 50-255)
Subject:
INCORRECT POSITIONING 0F CONTAINMENT BYPASS VALVES During preparations for performing a containment penetration integrity test, the licensee discovered that buth 3-inch valves on the bypass line around ths 48-inch ventilation exhaust valves were locked in the open position instead of the closed position.
These' valves are not listed on the Containment Integrity Valve Checklist which is completed prior to each startup.
The last Integrated Leak Rate Test of the Containment was made during March, 1978, and the valves would have been closed at that time.
In April, 1978, an efficiency test was made on filters in the bypass line and at this time the va've could have been incorrectly positioned.
The facility is shut down for a ten-week refueling and maintenance outage.
The licensee is investigating when and why these valves were placed in the incorrect position.
The resident inspector at the site will be following the licensee's review of this incident and the corrective actions taken.
The State of Michigan has been notified.
No Press Release by the licensee or NRC is planned at this time.
Region III received notification of this event from the licensee on September 14, 1979.
