o Prevent Recurrence 6

033 781201 F 7.0 G 3 N/A CH Instru Reactor scram on low vessel level.

An I&C Tech was working a work re-quest on the steam flow recorder (C32-R006), which was pegged high.

When lead to recorder was lifted, the 3-element level control loop was opened. The feed pump level control ran the feed pumps back, causing the vessel level to drop to the scram set-point. A meeting was held by plant management with the technicians in-volved to review the scram and deter-mine measures to prevent recurrence.

The details of the scram will be re-viewed with all 1&C Technicians in training sessions.

CAUSE & CORRECTIVE ACTION TO PREVENT RECURRENCE (cont'd)

Feasibility of installing Zener diodes in some plant current loops will be investigated. The diodes would allow current to bypass an instrument when the loop is broken at the instrument. This is a carryover from a November outage.

APPENDIX DOCKET NO. 050-0324 UNIT Brunswick No. 2 DATE January 3, 1979 COMPLETED BY Eulis A. Willis BRUNSWICK NO. 2 Monthly availability for Brunswick Unit No. 2 was 99.1% with only one major power interruption for the entira month.

December 1: A technician error on 11-30-78 caused a reactor scram which lasted 7.0 hours0 days
0 hours
0 weeks
0 months
into this reporting period.

The capacity factor for the month was 89%. For further 2xplanation of the outage, see the unit shutdown and power reduction log (Appendix D) for the month of December. Brunswick Unit No. I has no spent fuel in its pool at this time.
