STil $0-482/78-12 Appendix A fiOTICE OF VIOLATION Based on the results of the NRC inspection conducted on flovember 6-9, 1978, it appears that certain of your activities were not conducted in full ccm-pliance with the conditions of your flRC Construction Permit No. CPPR-147 as indicated below:

A.

Failure to Control Special Processes 10 CFR Part 50, Appendix B, Criterion IX requires that measures shall be established to assure that special processes are controlled and accomplished in accordance with applicable codes.

ASME Code,Section III, Subsection ND, paragraph ND-4412 requires that surfaces for welding shall be free of scale, rust, oil, grease, and other deleterious foreign material.

Contrary to the above:

The RECO procedures controlling welding do not specify require-ments for cleanliness and protection of welding surfaces. The IE inspector observed, on November 8, 1978, that mill scale was not being removed from the base metal prior to welding of the emergency fuel oil tanks.

This is an infraction.

B.

Failure to Follow Approved Procedures 10 CFR Part 50, Appendix B, Criterion V requires that activities affecting quality shall be accomplished in accordance with in-structions and procedures developed for these activities.

Daniel Procedures QCP-I-01, WP-XI-302, and ANSI 45.2.2 require that electrical equipment space heaters be continuously energized and checked weekly, and inspected and documented monthly; and that maintenance be performed in accordance with the frequency of manu-facturer's instructions (mis) or at least every six months.

These requirements also require QC surveillance of maintenance.

Contrary to the above, the following examples of failure to follow procedures related to storage of electrical equipment were noted:

During the plant tour, the IE inspector observed that the space heaters for all IE electrical equipment stored in place were de-energized.

Review of Daniel Procedures PJil-205H, Rev. 6 and RMI-S-002, Rev. 2 revealed that these procedures do not require that motor shaf ts be rotated as specified in the manufacturer's instruction book (I.B. 236930) which requires shaft rotation every thirty days.

Review of records and discusr ans with area engineers revealed that one motor shaf t was rotated every ninety days and the other one had never been rotated.

This is an infraction.
