Enclosed are the responses to your letter of September 19, 1979 in which you requested information on the involvement of certain companies with nuclear reactors that are now operating or under construction.
The information represents the best data obtainable in the time given to respond. Since not all dates requested were readily available, various dates are provided to help establish when the firms were likely to have been involved with the plants.
The following terms are used with dates provided.
CP - Construction Permit P0A - Preliminary Operating Authority POL - Provisional Operating License OL - Operating License As guidance, a firm's involvement usually begins at least one to two years before a construction permit date and five to seven years before any oper-ating license date.

Construction began in November 1975, and is still in progress (cps issued for both-12/75).

MARBLE HILL NUCLEAR GENERATING STATION UNITS 1 & 2 Public Service of Indiana Newberg is under contract to do the concrete construction and has other subcontractors working for them in some civil engineerina aspects of the job. Work on the project by Newberg began in August 1977 and continued until the Order Confi_rming Stop Work was issued on 8/15/79 (cps issued for both-4/78).

SALEM NUCLEAR GENERATING STATION Public Service Electric and Gas Company United States Testing Co. served as Operations Phase Quality Assurance advisor to the PSE&G Manager of Nuclear Operations including resident staffing (0L-for Unit 1-8/76).

CALVERT CLIFFS NUCLEAR POWER PLANT UNITS 1 & 2 Baltimore Gas and Electric Company United States Testing Co. provided a completely equipped on-site laboratory and personnel to serve as quality control arm on this nuclear construction project under the Bechtel Corporation.
Responsibilities included on-site testing of soils, concrete, Cadweld splices, welder coupons, and special materials.
Off-site backup testing provided for special materials and problems.
The United States Testin; Co. is serving as the initial Operations Quality Con +.rol Grcup for this nuclear fueled power plant, assisting in the develcpmelt and implementation of the Operations Quality Assurance Prcgram and training utility personnel to take over this function (OL Unit 1 - 7/74, OL Unit 2 - 8/76).

JAMES A. FITZPATRICK NUCLEAR POWER PLANT Power Authority of the State of New York U.S. Testinc Co., Inc. provided radiogr=phic examination and magnetic particle examination for Pcwer Authority of the State of New York during their first refueling and maintenance ou' cage.
Examinatic' was performed on newly constructed reboiler sys.;em and various in-service components.
United States Testing Co. as signed a representative to Power Authority of the State of New York to be responsible for final acceptance of raciooraohs (0L-10/74).

INDIAN POINT - UNITS 1, 2, & 3 Consolidated Edison Co. of New York United States Testino Co. provided the concrete cuality. control. function on Unit 1, and was assioned the role of quality control surveillance agent on Units 2 and 3.
Under direct assignment to con Ed, the company inspected concretino, and Caldweld splicing,. and reviewed contractors' quality control procedures for materials and fabrication methods. Under the direction of a senior project engineer, specialists are assigned to witness and inspect quality control tests tnd procedures at manufacturing plants.
Reports of each visit, with recommendations, are issued to the utility client.
The United States Testing Co. performed in-plant Qual'ty Control during modification of nuclear steam generators, including eddy-current testing of tubing for quality verification on Unit 3.
U.S. Testing Co., Inc. was under contract with Consolidated Edison Co.
of New York to perform ultrasonic examination of structural support bolts on-site (Unit 1 POL issued 3/62; now on long-term shutdown but not decommissioned. Unit 2 OL-10/71, Unit 3 OL-12/75).

PALISADES NUCLEAR POWER PLANT Consumers Poser Corporation The United States Testing Co. performed in-plant Quality Control kring the tube-plugging operation on the nuclear steam generators (0L 3. 'l).

MILLSTONE UNIT 1 Northeast Utilities Company United States Testing Co. performed verifiction activities on major piping modifications during refueling outage (POL-10/70).
7.
OYSTER CREEK Jersey Central Power & Light Company The testing and ipspection assignment on this nuclear generating station project was under Burns & Roe and. covered concrete and soils control.
Field and batch plant inspection was provided from a mobile job-site laboratory developed specifically for materials control on nuclear construction.
Off-site testing of materials, including concrete mix designs, was also provided (POL-4/69).

PILGRIM fiUCLEAR STATION Boston Edir,on Company U. S. Testing Co.
staffed and operated an on-site mobile quality control laboratory to provide the continuous monitoring of concrete at the automatic batching plant and during field placement.
Also, an on-site soils testing laboratory supported the field soil compaction
_ schedule.
Other tests and services were provided such as reinforcement steel testing, Cal 6eeld splice proof testing, testing of welds during welder certification (0L-6/72).

PPAIRIE ISLAND 1 & 2, MONTICELLO NUCLEAR PLANT Northern States Power C'ompany Under direct assignment with Northern States Power, United States Testing Co.
provided quality assurance surveillance on the nuclear steam supply system anc all components and structures associated with the engineered safeguards system.
Through a documentation system, complete records were made available of quality provisions for each component.
Recently, provided Vendor Qualification Surveys and Component Surveillance on replacement and spare parts procurement.
The program for Prairie Island and Monticello is designed to meet operations phase requirements at mininun cost to the utility (Monticeli] POL-9/70; Prairie Island Units 1 & 2 OL-8/73 & 10/74, res oective.y).

KEWAUNEE SUCLEAR STATION Wisconsin Public Service Company As a supporting am. to Pioneer Service & Engineering Company, the United States Testing Co. provided vendor plant cuality assurance surveillance on selected components within the nuclear steam supply system and other critical components.
Performed in conjunction with the Prairie Island assignment (0L-12/73).

These services encompass balance-of-plant components and involve manufacturer's facilities surveys, development and maintenance of procurement contract files for use in vendor surveillance activities and by quality assurance engineers.
Surveillance is performed at vendor shops on purchased components.
These activities include evaluation of nondestructive tests, adherence to purchase order and specification requirements, and final acceptance (OLs issued-4/74 & 2/78).
14 TURKEY POINT NUCLEAR PLANT UNITS 3 84 Florida Power and Light Company U. S. Testing Co. is currently providing OC Inspection and Outage Support at these units (OLs issued-7/72 6. 4/73).

TON P0h'ER STATION UNITS 1 & 2 Illinois Power Company Under direct assign ent to Illinois Power Company the United States Testing Co. provides resident inspection personnel, operating from a fully equipped on-site laboratory, for the quality control testing and inspection of soil fill material, concrete aggregates, designing, batching and mixing of concrete, testing of reinforcing steel and Cadweld spliced bars plus complete nondestructive testing and examination by radiography, magnetic particle, dye penetrant and ultrasonic techniques.
Additionally, the company provides, general Quality Assurance Consulting Services..
U.S. Testing Co., Inc. is presently under contract to Illinois Power Co. to perform all on-site NDE examinations, establish NDE procedures and perform training and certification of Illinois Power on-site NDE pe rs onnel. Radiography has been performed on dr.y well wall, weir wall, storage tank, radioactive waste tank and welders coupons.
In addition, magnetic particle and liquid penetrant techniques have been utilized on various components (cps issued for both units-2/76).

PERRY NUCLEAR POWER PLANT UNITS 1 & 2 Cleveland Electric Illuminating Company Under direct assignment to the utility, the United Stater Testing Co.
has the responsibility for equipping, staffing, and operating the on-site testing laboratory to serve as the utility's site organization Q.C. ann.
Responsibilities are for civil materiils, reinforcing steel and splices plus welder quaiffication samples.
Included in the program are materials qualification tests, concrete mix designs and all related off-site testing where necessary.
U.S. Testing Co., Inc. was under centract with Kaiser Engineering, Perry, Ohio, to establish and perform ultrasonic examinations in accordance with ASME Sec. III on imbedded plate located in reactor housing wali.
In addition, a United States Testing representative conducted classroom instruction on ultrasonics for on-site personnel from Kaiser, Gilbert!s, CEI and Newport News (cps issued for both units-5/77).

LIMERICK NUCLEAR GENERATING STATION UNITS 1 & 2 Philadelphia Electric Company United St3tes Testing Co. provided an on-site laboratory and field inspection personnel for quality control of soils, concrete, reinforcing steel and Cadweld splice testing.
U.S. Testing Co., Inc. was under contract with Bechtel Corporation, Sanatogo Branch for on-call NDE services, and performed services in the areas of radiogrLphic and ultrasonic examinations at Limerick (cps issued for both units-6/74).

MARBLE HILL NUCLEAR STATION UNITS 1 & 2 Public Service Company of Indiana The United States Testing Co., under contract to the utility, has the responsibility of providing and operating a fully equipped on-site laboratory.
Corporate project leaders develop quality control procedures for on-site and off-site testing of materiels, calibration of test equipment and material, and equipment receipt. inspection.

This program consisted of quality assurance audit responsibilities for all on-site equipment and construction practices and all off-site manufactured equipment for the nuclear steam supply system (CP-5/74).

SAN ON0FRE NUiLEAR P0h'ER PLANT UNITS 2 & 3 Southern California Edison Company The United Statt esting Co. has the responsibility for equipping, staffing, and operating the on-site laboratory for civil materials testing during construction of the plant.
The program includes testing and inspection of concrete materials, reinforcing steel and splices, and welder qualification samples.
Related off-site testing is also provided where necessary (cps issued for both units-10/73).

MIDLAND NUCLEAR PLANT UNITS 1 & 2 Consumers Power Company The U. S. Testing Co. is under assignment by bechtel Power Corporation to provide an on-site laboratory and resident personnel for quality control testing and inspection of soil fill material, concrete, aggregates and reinforcing steel including Cadweld splices.
Off-site laboratory testing support is also provided for chemical analysis and special tests for properties of materials.
On-site Quality Assurance audit assistance is provided to the utility by the United States Testing Co. during the construction phase (cps issued for both units-12/72).

SUSQUEHANNA STEAM ELECTRIC STATION UNITS 1 & 2 Pennsylvania Power and Light Company The U. S. Testing Co. is under contract to Bechtel Power Corporation to provide an on-site laboratory and test equipment.
Resident inspection personnel provide the quality control testing and inspection of construction materials including soil, fill, aggregates, batching and mixing of concrete, and testing of reinforcing steel including Cadweld spliced bars (cps issuod for both units-ll/73).
