APPENDIX A NOTICE OF VIOLATION Jersey Central Power and Light Company Docket No. 50-363 License No. CPPR-96 Based on the results of the NRC inspection conducted on June 25-27, 1979, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. CPPP,-96 as indicated below. This item is an infraction.
10 CFR 50, Appendix B, Criterion V, states, in part, that:
" Activities affecting quality shall be prescribed by documented instructions, pro-cedures, or drawings... and shall be accomplished in accordance with these instructions, drawings, and procedures..."
The licensee's Quality Assurance Plan for Forked River Nuclear Station, states, in part, in Paragraph 13.1:
"CPUSC has the overall responsi-bility for assuring that the FRNS Quality Assurance Program has adequate provisions for the control of handling, storage and shipping..."
GPUS0 QC Procedure FR-13-01, Rev. O, states, in part, in Paragraph 5.1 that GPUSC OA is remonsible for " Performing and documenting surveil-lance inspection of preventive service and storage program" and " Review and appron1 of storage and servicing instructions."
Field Shutdown Procedure, FSP l-05, Paragraph 6.1.2, requires that the original ESHC (Equipment Storage History Card) be forwarded to GPUSC for review and approval and states, in part, "GPU... shall signify approval by c'gnature and date at the bottom of the card."
Conttary to the above, on June 25, 1978, iettain ESHC's written during May and June, 1979, had not been approved and surveillance ir.spections of the preventive service ano storage program had not been ',onducted since late March, 1979.
