This refers to the discussions between Mr. Crouse and Mr. Murray and other members of your organizdtion and Messrs. Warnick, Reyes and Smith of this office on October 17, 1979, regarding the reactor trip and loss of offsite power that occurred on October 15, 1979.
With regard to the matters discussed, we understand that you will determine the cauce of failure and identify the corrective actions to prevent recurrence of the following:
1.
Initiating cause of the reactor trip.
2.
Failure of Breakerc HXO2A and HX02B to close on the initial attempt.
3.
Muffler failures on 345 KV breakers.
4.
Inability to start RCP's on the first attempt because of CCW interlock.
5.
Failure of the CCW 2 Pump and the SW 3 Pump to be automatically sequenced during the loading of emergency diesel generator 2.
Further, it is our understanding that these actions will be completed by your staff and reviewed by the NRC onsite representatives before the Davis-Besse Nuclear Station is allowed to proceed to Mode 2.
If these understandings are incorrect, please notify this office immediately.
