CONDITloNS 1.

The licensee shall obtain all information from the foreign exporter aecessary to complete the shipper's porti'on of Fom NRC-741, " Nuclear g'

Material Transactions Report."

If the licensee is the receiver of the material he shall record both the shipper's,and receiver's infomatiori on the Fom NRC-741 and transmit pursuant' to Section 70.54 of 10 CFR Part 70.

If the licensee is not the receiver of the material, he shall provide the shipper's infomation to the receiver of the material for inclusion on the receiver's Fom NRC-741.

The licensee shall notify the appropriate regional Safeguards Branch, Office of Inspection and Enforcement, immediately, by telephone or h

telegram, if entry of any import shipment authorized by this license is refused by tne U.S. Customs Service.

Each time material is imported, the licensee shall show the original u

of the license to U.S. Customs as authority for the import.

The l4 original of the license showing the imported quantity shall be surren-4

'dered to U.S. Customs when the final transaction under the license is completed.

Except for the final transaction, a copy of the license Q

showing the imported quantity shall be surrendered to U.S. Customs'for each shipment.

All surrendered documents shall be marked for immediate transmittal to the Office of International Prearams, U.S. Nuclear Regulatory Commission, Washington, D.C., 20555.
