Appendix A NOTICE OF VIOLATION 4

Southern Illinois University License No. 12-01109-08 Based on the inspection conducted on September 18 and 19, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.

Item i is an infraction, and item 2 is a deficiency.

License Condition 16 states, in part, that licensed material shall be used in accordance with statements, representatlons and prc edures contained in an application dated July 24, 1973. Section 5 of the

" Handbook of Radiological Operations" requires that bioassay measure-ments be made when personnel work with unsealed sources of radioactive material exceeding at any one time the investigation level as defined in the Operations Handbook.

Contrary to this requirement, a bioassay w asurement was not made for an individual working with 1-2 millicurie ansealed quantities of iodine-125 on two occasions between October 1978 to the date of this inspection. These quantities are above your investigation level for iodine-125.

Title 10, Code of Federal Regulations, Part 20.203(e) states, in part, that each area or room in which licensed material is used or stored in excess of certain quantities shall be conspicuously posted with a sign bearing the radiation caution symbol and the words:

Caution Radioactive Material (s).

Contrary to this requirement, no such sign was posted on any side of the radioactive waste storage garage prior to September 19, 1979, the second day of the inspection.
