2 DETAILS SECTION A.

Person Contacted Authorized Nuclear Inspector (ANI) National Board Commission No. 8460 B.

General The ANI is employed by Hartford Steam Boiler Inspection and Insurance Company (HSB, I&I Co.), and reports to the Atlanta Regional Office. The ANI provides inspection service on a notification basis to Colt Industries, Trent Tube Division of Crucible, Inc., plant located at Carrollton, Georgia.

Qualifications (ANSI 626-1974, paragraph 4.1)

The ANI provided the following information with regard to his qualifications.

1.

Age (626, 4.1)

Thirty-five (35) years; born July 1, 1944.

2.

Education (626. 4.1.2)

High School graduate.

3.

Work Experience and Demonstrated Ability (626, 4.1.2, 4.1,3 and 4.1.4) a.

Sonar Technician, U. S. Navy, 4 years.

Completed TVA's Steam Power Plant Operator school,18 months.

b.

Completed courses for AI and ANI Training as designated in HSB I&I Company's " SIS Training and Development Program" and HSB NDE course for UT, 8 months.

c.

Inspected at CB&I Company's Birmingham ASME Section III vessel shop and at TVA's nuclear power plant construction sites.

4.

Inspection Expertise, Knowledge of Codes, and Examination (626, 4.1.5, 4.1.6, 4.1.10)

Successfully passed the National Board's special examination leading to issuance of National Board Commission No. 8460, State of Ohio Commission as Special Inspector and State of Tennessee Commission No. 2236, and was certified as an ANI in March, 1979.

Knowledge of QA Manuals, QC Programs, Procedures, and Records (626, 4.1.7, 4.1.8, and 4.1.9)

This ANI participated in five (5) ANI supervisor's semi-annual audits of manufacturers' pressure vessel shops in Birmingham, Alabama.

D.

Performance of Duties (626, 4.2) 1.

General Information Through discussions and review of records, the following information was obtained with respect to performance of ANI duties identified in ANSI 626-1974.

On receipt of an "NPT" order by Trent Tube, the plant management provides the ANI with records of:

raw material including weld wire releases, Certified Material Test Reports and Mill Orders (production routings) for review acceptance and assignment of ANI hold points on the Mill orders, and sign-off release of the Mill Orders to production. The ANI has been providing inspection services on a notification basis since May, 1979, which involved eleven (11) visits to the Trent Tube plant.

Specific Information a.

Discussion established that the ANI has reviewed Trent Tube's ASMI Certificate of Authorization to use the NPT symbol. He indicated that he has: observed (monitored) dye penetrant tests, radiographic examinations and dimensional inspections; also, he has reviewed the released production Mill Order routings.

The ANI indicated that he has checked and found that Trent Tube has the necessary code books, addenda, and code cases on file.

(626, 4.2.3) c.

Verification of availability of Design Specifications and Stress is not applicable to this manufacturer of welded piping.

(626, 4.2.4) d.

The ANI reviews Certified Material Test Reports (CNTRs) and Mechanical Test Reports to verify that all materials comply with applicable ASME Code requirements.

The following ANI activities have not been performed by this ANI due to the limited number of inspections he has performed since assuming inspection duties on a notification basis in May, 1979, at this plant:

4 (1) Verifying transfer of identification prior to cutting of SA-358 piping into two (2) or more pieces.

(2) Verifying manufacturer's personnel are examining all cut edges of plate, sheet and strip.

(626, 4.2.7)

(3) Verifying that all welding procedures conform to the ASME Code Sections III and IX.

(626, 4.2.8)

(4) Verifying that welders and welding operators are properly qualified.

(626, 4.2.9) f.

Verification of welding controls during construction and welded repairs weArol during construction are not applicable to this shop.

, 4.2.10 and 4.2.11) g.

The ANI verifies that required heat treatments are performed and documented by review of heat treatment furnace charts and material specifications.

(626, 4.2.12) h.

The ANI indicated he has observed dye penetrant and radiographic examinations on in process piping welds but has not reviewed NDE procedures and personnel qualificatica due to time limitations common to his inspections.

(626, 4.2.13) i.

The ANI witnesses hydrostatic tests which are identified as Hold Points on the mill order travelers.

(626, 4.2.14) j.

The ANI reviews the entries on each Code Data Form prior to signing it, to assure that the responsible manufacturer's representative has signed it and the entries are correct.

(626, 4.2.15) k.

Prior to applying the code symbol to the pipe, the ANI verifies that his inspections are completed, and signs the Code Data Form to verify that to the best of his knowledge, the item is in compliance with the code. He then observes the application of the NPT symbol by the electro-etch, vibro-etch or other methods.

(626, 4.2.16) 1.

The ANI inspects such items as materials and final dimensions reported on the Code Data Report.

(626, 4.2.17) m.

Review established that the ANI maintains a bound log book

" SIS Daily Inspection Record" of his inspection activities.

The two (2) log-books available were for the period of June 4, 1979, to present (August 28, 1979), and for the period of July 19, 1976, to May 18, 1979. Review also verified that several different ANIS inspected at Trent Tube during the above periods.
