APPENDIX A Northeast Nuclear Energy Company Docket No. 50336 Based on the results of an NRC inspection conducted on March 17 - April 30, 1979, it appears that one of your activities was not conducted in full com-pliance with the conditions of your NRC license as indicated below.
This item is an infraction.
Technical Specification 6.11 states:
" Procedures for personnel radiation pro-tection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure."
Health Physics Operating Procedure HP 903/2903 Radiation Work Permits, Revision 7, dated March 15, 1979, paragraph 5.11.2 states:
" Personnel shall not deviate from instructions of the Radiation Work Permit without authorization from the Health Physics Supervisor / Designee and with his notation on the RWP."
Contrary to these requirements, on April 5,1979, the protective requirements of RWP 791479 were not followed.
This resulted in internal contamination of two workers with radioactive material.
A response to this item is not required.
The corrective actions taken of the licensee have been reviewed by the inspector.
