MEMO FOR FACSIMILE TRANSMISSION TO CONFIRM PROMPT REPORIABLE OCCURRENCE 335-79-30 This memo confirms our telephone notification of the subject 14-day reportable occurrence made on September 18, 1979 to Mr. J. Dyer of your office.
During the inspection / evaluation of pipe restraints / supports in accordance with NRC I & E Bulletin 79-14, two restraints on ECCS piping systems were found to be in non-conformance. Restraint SI-676-429 on line 4"-SI-416 (vertical riser section) was found as a U-bolt type restraint which would prohibit movement in both the North-South and East-West directions. By design it should permit movement in the East-West direction.
A restraint on line 6"-SI-464 (horizontal section) which is designed to prohibit lateral and vertical movement was found missing.
In its place was pipe hanger SIH-99 which is suspended from the ceiling above.
Both restraint errors have been corrected. The U-bolt at restraint SI-676-429 has been replaced with a unidirsctional restraint that parmits East-West movement New restraint SI-868-6099 has been fabricated and installed on line 6"-SI-464 to prohibit vertical and lateral movement.
