IDENTIFICATION OF OCCURRENCE:
Safety Features Actuation System (SFAS) Channel 2 Containment Radiation Detector RE 2005 spiked The unit was in Mode 1, with Power (m.7) = 2758, Conditions Prior to Occurrence:

Description of Occurrence: On August 11, 1979 at 1430 hours0.0166 days <br />0.397 hours <br />0.00236 weeks <br />5.44115e-4 months <br />, SEAS Channel 2 Contain-ment Radiation Detector RE 2005 spiked (tripping the SFAS Channel 2 Containment Radia-tion Bistable) and returned to normal.
Several spikes occurred in the fifteen minutes prior to the channel trip. Upon receiving the trip, RE 2005 was declared in-operable. This placed the unit in the Action Statement of Technical Specification radiation in-3.3.2.1.b9 which requires the operability of all four SFAS containment strument strings in all modes. The SFAS Channel 2 containment radiata.on bistable was left in the tripped state as required by the Action Statement.
The containment radiation instrument strings associated with the other three SEAS channels were opera-ble, meeting the minimum operational requirements.
Designation of Apparent Cause of Occurrence:
The tripping of SEAS Channel 2 was The cause of the caused by the spiking of Containment Radiation Detector RE 2005.
spiking has been attributed to vibration in the area of the detector waich causes flexing of the ion chamber connectors to the high and low range boards. Victorcen has indicated this flexing can generate a small voltage which would cause the spiking problem; testing in our Instrument and Control chop substantiates this.
Analysis of.0ccurrence:
There was no danger to the health and safety of the public or to station personnel. The containment radiation monitoring instrument strings associated with the other three SEAS channels were operable.
RE 2005 failed in the high (safe) direction.
Corrac.tive Action: A neu detector was calibrated and installed per MWO-IC-383-79.
