APPErlDIX A fl0TICE OF VIOLATION Baltimore Gas and Electric Company (BG&E)

DPR-53 DPR-69 Based on the results of the flRC inspection conducted on February 12-16, 1979, it appears that certain of your activities were not conducted in full compliance with flRC regulations and the conditions of your flRC Facility License flos. DPR-33 and DPR-69 as identified below.

Item A is categorized as an Infraction.

The remaining items are Deficiencies.

A.

Section 5.5 of the Environmental Technical Specifications (ETS) for Units 1 and 2 require, in part, that systems which are directly involved with the fulfillment of the ETS be operated in accordance with approved operating procedures, and that all written procedures and changes thereto, which pertain to the specifications in Sections 3.0 and 4.0, be reviewed by the Environmental Engineering Group, Electric Engineering Department and approved by the Chief Environmental Engineer prior to implementation.

Contrary to these requirements, procedures used to fulfill several of the activities pertaining to Section 3.0 of the ETS for Units 1 and 2, including analytical, sampling and radioanalytical procedures of all the required environmental media, were not reviewed and approved as specified above.

B.

Section 3.2.1 of the ETS for Units 1 and 2 requires, in part, that the analytical techniques be such that the Lower Limits of Detection (LLD) listed in ETS Table 3.2-2 be achieved.

The listed LLD for Sr-89 and Sr-90 in fish are 40 pCi/Kg (wet) and 8 pCi/Kg (wet),

respectively.

Contrary to these requirements, fish sampled during the fourth quarter of 1978, were not analyzed with analytical technique so as to achieve the above specified LLD.

C.

Section 3.2.1 of the ETS for Units 1 and 2 requires, in part, that vegetation samples, including predominant food crops be sampled at harvest time, from one onsite location and two of the nearby farms as shown in Figure 3.2-1.

Contrary to these requirements, the two sampled food crops, corn and soybeans, were not collected at harvest time during 1977 and 1978.

These samples were collected during November and December.

Contrary to these requirements, vegetation samples, including food crops were not collected from locations No.15 and 16 during 19/7 and location No.15 during 1978. These locations are as shown in Figure 3.2-1.

D.

Section 2.2.1 of the ETS for Units 1 and 2 requires that the pH be in the range of 6.0-9.0 prior to release into the circulating water discharge.

Contrary to this requirement, on December 16, 1978, waste solution which was outside the pH range of 6.0-9.0, was released from the neutralization tank and discharged to the circulating water flow, n., o,.
