On June 11, 1979, the services of

Ind."A"

were contracted by the Henry

J. Kaiser Company at the Wm. H. Zimmer Nuclear Power Station throug: athr

Services Group, Inc. The services of such employees are considered temporary

in nature, and they are considered to be probationary employees for a period

of two to three months. After a period of testing and training,

Ind."A"

was qualified as an inspector and issued an inspection stamp.

Over the past several weeks,

Ind."A"'s

supervisors, who were monitoring his

work, determined that

Ind."A"

was performing inadequate inspections and

that his performance was not in accordance with the standards required by

the Nuclear Regulatory Commission.

,

As a result of this determination, it was decided that the services of

Ind . A"

were no longer required, and the contract with Butler Services,

Inc. for his services was cancelled on August 14, 1979.

Henry J. Kaiser

Company did not employ

Ind."A"

so, therefore, the Company could not term-

inate him. Wnether or not

Ind."A" is stili employed by Butler Services,

Inc. is beyond our knowledge and/or control.

At the present time, our

Quality Assurance Department is in the process of reverifying the inspections

performed by

Ind. "A" during the short period of time he worked at the

Zimmer site.

SUBJECT: TERMINATION OF CONTRACT EMPLOYEE

Bea Palermo, Personnel Manager at Butler Services, was contacted approximately

8:00 AM this date, concerning Individual"A"s performance in the area of visual

inspection.

Indiv."A"

was contracted by Kaiser from Butler Services Group,

Inc. on June 11, 1979.* He completed his reading list, passed his Visual Weld

Ir.spection Test and Liquid Penetrant Test on June 15, 1979

He was issued an

irs;ection Stamp on that date.

inspections as itemi2ed below reveal that he did not perform in

Iradequate

accordance with his qualifications as reflected by his resume and certifi-

cations.

1) Welder qualification was expired and the inspector should have

identified this condition at the time tacks and fitup inspection

was made, instead he allowed the unqualified welder to complete the

weld.

Nonconformance report E1985 was issued.

2)

Inspector allowed welders qualified for weld procedure 3.1.51 weld

to 3.1.21 without approval from the Welding Engineer, and without

3.1.21 being cross referenced in QACHI G-12, as an equal to 3.1.51.

3)

Several fitups in the suppression pool modification contrary to

the designs specification were accepted by the inspector.

a)

Backing strips on plates 2C, IC and 6C were to short and

left gaps of the sizes 1" x 1/8", 1" x 1/4", and i 1/4" x

1/4" respectively. The gaps will not allow full penetration

selds as designed or allowed by DDC S-1701.

1/2" liner plate was not ground down to allow the proper

fitup, instead the backing strip was tacked to the, edge of

the liner plate allowing space between backing strip and

liner thus preventing full penetration weld.

Paint and heavy rust was left on backing strip.

Tacks on backing strips were cracked.

He also was not receptive to instructions and clarifications provided by

supervision.

It was expressed to Bea Palermo that Kaiser would like to

terminate it's contract as soon as post.ible.

Approximately 1:00 PM, Bea Palermo called to inform Kaiser QA that

contract was terminated this date.
