This refers to the telephone conversation between you and Mr. W. C. Seidic of this office on September 7,1979, concerning the inadvertent omission of shear tie reinforcing steel from the 32nd lif t of the Comanche Peck, Unit No. 2 Reactor Containment Building (RCB) exterior wall just below the springline.
Based on the above conversation, we understand that you have or plan to:
1.
Issue an order, effective September 7,1979, to stop placement of safety-related concrete in the Unit No. 2 RCB exterior wall and dome.
2.
Review in detail the Gibbs & Hill (A/E) engineering analysis which concludes that the omission of the shear ties does not impact signif-icantly on the structural integrity of the RCB and that the corrective action taken satisfies the original design requirements.
3.
Analyze and correct the breakdown in the QA/QC program which allowed the omission of the reinforcing steel to occur and identify corrective action that has been taken to minimize recurrence of this type of QA/QC breakdown.
4.
Provide this office with a written report of the omission of the shear ties and discuss the design changes you have made to the RCB exterior wall as a result of this omission together with your engineering evaluation of the design change resulting from the omission of the above referenced reinforcing steel.
5.
Contact the Region IV of fice prior to resumption of placement of safety-related concrete in the Unit No. 2 RCB exterior wall and dome.
